01 · The morning promise
Make a promise. Keep a promise.
She asked for a total line at the bottom of her intake. What she needed, from her ask the day before, was both intakes live on the website as forms people fill in.

02 · Live totals
Every tap adds up, as they answer.
Each person taps 1 to 5 for every statement. The section total counts as they go, out of 40, and shows how many are answered.

On a computerSection 01, all eight answered: 22 / 40

Sample taps in a test browser. Nothing was sent.
03 · The grand total
Four sections. One number out of 160.
Under section 04 a grand total adds 01 to 04 and lists each section beside it. The totals go to her with the answers, so nobody adds anything up by hand.

04 · New Client Intake
Eight pictures of pages. One real form.
Her New Client Intake was eight page images in a Word file, with nothing to type into. Every question a client answers became a real field, in her words, from Personal Information to Your Intention.

BeforeEight pictures of pages in a Word file

Practitioner Notes stay off the online form. That page is hers to fill in after a session.
05 · The tooth chart
Tap the tooth. No pen needed.
Her dental page asks people to circle teeth on a diagram. Online they tap the tooth numbers instead, and the marked teeth come through with the rest of the answers.

06 · Word for word
Her words, exactly. On screen and on paper.
- The wording on both forms matches her documents word for word.
- Her print Word file got TOTAL rows in her own style: each section out of 40, a grand total out of 160.
- A PDF preview of the print file came with it.
- Every file on the site was pulled back after the push and matched byte for byte.

The newsletter is shown as built, as a preview; the sign-up boxes are staged, not live, and no contacts are shown. Form screens show sample taps in a test browser; no client answers are shown and nothing was submitted. The intake content is hers; no health claim is made.
